  • We Hear U | Mountain Youth

We Hear U What is it? This free program offered at Mountain Youth has been adapted for the Eagle County community from Active Parenting of Teens, an evidence-based program designed to help families survive and thrive. It focuses on active communication, parenting styles, logical consequences, mutual respect, and more. How does it work? Once a referral is made, a facilitator will meet with one or more families in a 3hr core session at a time that works well for the families. A meal is provided, as well as childcare if needed. After the core session, up to three 1hr follow up sessions can be scheduled. These follow up sessions are usually two weeks apart to see how things have progressed, discuss what the family is working on, and provide additional accountability and support. Most of referrals for We Hear U come from counselors within the Eagle County School District, but community members can sign up for the program themselves, as well. While our facilitators are not licensed counselors, they are certified in training on parenting and communication, and can provide sessions in both English and Spanish. How do I sign up or make a referral? Fill out a referral form to get started.

  • Collaborative Management Program | Mountain Youth

Collaborative Management Program Where do families turn when youth are experiencing challenges that feel overwhelming and require several providers to deliver services? Collaborative Management Program (CMP) might be the answer. CMP facilitates coordination of services for youth involved in multiple systems by engaging the youth, their families and other partners. The goal is to improve positive outcomes for the youth. To qualify for Eagle County CMP services, the youth must: Reside in Eagle County Be between the ages of 0 and 18 years old Be involved in multi-systems (e.g. mental health system, health care system, school system – see list of IOG partners which comprise the systems) IOG Partners 5th Judicial District Probation Bright Future Foundation 5th Judicial District Courts Mountain Youth Eagle County Public Health Family Advocate Eagle County School District Mountain Family Health Roaring Fork School District Youth Power 365 Mind Springs Health Eagle Valley Behavioral Health Division of Youth Services The Hope Center Rocky Mountain Health Plans CASA of the Continental Divide Youth Detention Continuum (CYDC) Early Childhood Partners Eagle County Department of Human Services Referrals for youth can be made by these IOG partners. For English referral forms please click here. For Spanish, please click here. For assistance, contact Jessica Beegle . After a referral is submitted for an eligible youth, families and the youth attend a family meeting. CMP plans these meetings with community agencies that provide services for the youth. The meeting is family-focused, where their voices are the most important. The youth and family make the decisions on what services they will use based on recommendations made by providers. History In collaboration with IOG Partners, Mountain Youth operates the Eagle County CMP through a contract with Eagle County Department of Human Services. CMP is partially state-funded by the Colorado Department of Human Services and several local funding entities. In 2004, the Colorado State Legislature established CMP (House Bill 04-1451) with the purpose of supporting counties in facilitating coordination of services for multi-system involved youth in order to avoid duplication and increase positive outcomes for youth.
